Brodie abscess is most commonly found in the tibia, as is the case here. This focus of chronic osteomyelitis demonstrates the classic features of involucrum formation with a draining cloaca and sinus tract to the skin.

Brodie abscess is a subacute or chronic intramedullary abscess due to chronic osteomyelitis. Typical features in this case include well-circumscribed lytic lesion, cortical thickening, and smooth (non-aggressive) periosteal reaction.

The penumbra sign is a thin rim of granulation tissue that lines the abscess cavity in cases of subacute osteomyelitis. It has a characteristic of a high signal on unenhanced T1 WI compared to the central hypointense fluid-filled abscess cavity and the surrounding bone marrow edema.

He was managed conservatively with 2 weeks of IV antibiotics and was discharged home after his limp resolved and repeat MRI showed no further signs of abscess. He continued 6 weeks of broad-spectrum antibiotics in the community.

MRI features of a Brodie abscess of the tibial metaphysis with a "penumbra sign". The penumbra sign is seen as a rim of vascularized granulation tissue surrounding a bone abscess cavity. It elicits a higher signal intensity than the cavity itself on T1, hypointense on T2 with enhancement on postcontrast sequences.

Unenhanced CT of the right knee demonstrates a lucent ovoid lesion centred at the proximal tibial metaphysis and extending into the epiphyseal plate. There is a narrow zone of transition with somewhat sclerotic borders and a central calcific focus.
